Clinic medical services by appointment only.Intake Process:Individuals must come to the clinic on registration days and bring the following documents:* Picture ID* Proof of Address* Proof of family Income for the last four weeks.Eligibility:* Children and adults must live in Miami-Dade County.* Must be at or below the 200% Federal Poverty Level.* Must not have any type of health care insurance or coverage.* Cannot have Medicaid or any other federal or state healthcare coverage.
